Answer 2 PID. PID is the most likely diagnosis in a patient who presents with the triad of fever, pelvic pain, and purulent vaginal discharge. Fitz-Hugh-Curtis syndrome is a complication of PID presenting with right upper quadrant pain. It is a perihepatitis caused by the spread of bacteria (either Neisseria gonorrhoeae or Chlamydia trachomatis) from the pelvis to the liver capsule. Ovarian torsion would present with acute pelvic pain but not a vaginal discharge. Ectopic pregnancy would present with an adnexal mass and vaginal bleeding. Click here to return to the questions
